Registration Forms & Requirements for Kindergarten students only!

Registration at Loma Prieta Joint Union Elementary School District (LPJUSD) for the 2025-2026 school year opens on
Monday, March 3, 2025.

Registration must be completed online.  Prior to completing the online registration, please ensure you have all of the following required documents available to you. The necessary forms are linked below for your convenience.

If you are unable to print the required forms, please visit district Registrar Lisa Sullivan in our C.T. English Middle School office for assistance.

• Photocopy of your child’s original birth certificate or passport.

  Students born on or before 9/1/2020 are eligible to attend Kindergarten.

• Photocopy of your child’s immunization record, as of the registration date. Please see the 'Guide to Immunizations' document below to ensure your child has received all of the mandatory immunizations to start school in California.

• Photocopy of any shared custody arrangement for your child(ren).

  Legal Custody Agreement at the time of registration is required.

• Oral Health Assessment Form, completed by your child's dentist.

• Kindergarten Health Assessment Form, completed by your child's doctor

• Statement of Residency Form. If you are unsure as to whether you live within the district boundaries of LPJUSD, please contact our district office at (408) 353-1101.

• 3 Proof of Residency documents. Please see the 'Proof of Residency Requirements' below to ensure you have one supporting document from each of the three required sections.

• Kindergarten questionnaire.

The registration of your child at LPJUSD is not complete until all registration forms have been validated by our registrar and health clerk.

We strongly recommend that you register as soon as you are able, so that you begin to receive communication from our Principal before school starts on August 15, 2025. Our district calendar for the 2025-2026 school year is linked below for your convenience.

If your child has not yet received the required immunizations, or has not yet visited their doctor or dentist for the required health assessments, we recommend you make appointments as soon as possible to avoid the busy schedules during the summer months.

Registration is most compatible with desktops or laptops, not mobile phones.
